瘢痕癌8例诊疗分析

The analysis oftreatment of 8 patients with Marjolin ulcer

目的探讨瘢痕癌的临床特点与治疗方法。方法回顾性分析近12年来8例瘢痕癌患者的临床资料。结果 8例均属慢性瘢痕癌,其中溃疡型5例,菜花型3例;除1例手背瘢痕癌外,其余患者的移植皮片或皮瓣100%成活,2例截肢伤口Ⅰ期愈合。8例患者分别于术后4个月～6年获得随访,1例于术后6个月因颅内转移死亡,1例于术后5年死于其他疾病;1例足跟部瘢痕癌于术后4个月复发并侵噬跟骨,行小腿中段截肢,其余5例局部无复发,功能外形良好。结论病理检查结果是确定诊断与治疗方案的重要依据,手术仍是瘢痕癌的首选治疗方法。

Objdctive To study the treatment of Marjolin ulcer and clinical characteristic.Methods The clinical materials from 8 patients with Marjolin ulcer were summed up and analyzed during late 12 years. Results All patients were diagnosed as chronic Marjolin ulcer.Among them,5 cases were ulcer type and cauliflower-like tupe 3 cases.All skin grafts and skin flaps survived except for one case of Marjolin ulcer on scalp.The wounds of amoputation healed on the first stage.8 patients were followed up respectively from four months to six years post-operation,and two caese died from other disease and the recurrence appeared in one case.Conclusion The result of pathological examination is the important basis of diagnose for Marjolin ulcer. Surgical treatment is still the first selection to Marjolin ulcer.
