子宫颈鳞状细胞癌VEGF表达和MVD测定的临床病理意义研究

摘要 目的 :探讨血管内皮生长因子 (vas cularendothelialgrowthfactor ,VEGF)表达和微血管密度 (microvesseldensity ,MVD)在子宫颈鳞状细胞癌中的临床病理意义。方法 :应用EnVision二步法检测 92例子宫颈鳞状细胞癌VEGF的表达和MVD。结果 :正常子宫颈鳞状细胞有 2 /10表达VEGF ,92例子宫颈鳞状细胞癌中VEGF表达率为71 7% ( 66/92 ) ,宫颈原位鳞状细胞癌、高、中、低分化鳞状细胞癌VEGF表达分别为6/10、65 0 % ( 13 /2 0 )、75 0 ( 15 /2 0 )、80 0 %( 3 4/4 2 )。且VEGF表达在正常子宫颈鳞状细胞和宫颈原位鳞状细胞癌间差异有统计学意义 ,P =0 0 0 5 7。宫颈原位鳞状细胞癌、高、中、低分化鳞状细胞癌MVD值分别为 2 2 4± 7 9,2 7 9± 9 1,3 0 7± 11 8,3 4 6± 14 3 ,VEGF表达和MVD皆与宫颈鳞状细胞癌的病理分级和临床分期密切相关 ,P<0 0 5。结论 :检测VEGF的表达对宫颈早期鳞状细胞癌的判断可能是有用的指标 。 OBJECTIVE:To investigate the correlation between the expression of VEGF and MVD values and pathological grade and clinical staging of squamous cell carcinoma of cervix.METHODS: Ninety-two patients with squamous cell carcinoma of cervix treated from 1998 to 2002 were,and the expression of VEGF and MVD values were examined on the paraffin specimens by using EnVision immunohistochemical method.RESULTS: Twenty percent of normal squamous cell of cervix was expressed VEGF, and VEGF was expressed in 66 specimen(71.7%),the positive percentage of VEGF expression was 6/10,65.0%(13/20),75.0(15/20)and 80.0% from carcinoma in situ to low-differentiated,respectively. There was a high significance,P=0.005 7 on expression of VEGF between the normal squamous cell of cervix and its carcinoma in situ. MVD values ranged from 22.4±7.9 to 34.6±14.3, and the more MVD counted, the highlier the cervix malignancy progressed. There was an intimate correlation between the expression of VEGF and MVD values and pathological grade and clinical staging of squamous carcinoma of cervix.CONCLUSIONS:VEGF expression probably is an useful marker in judging early squamous carcinoma of cervix, and VEGF expression and MVD value are of some instructive significance in judging the histopathologic grade and clinical staging of squamous carcinoma of cervix.
