第二产程剖宫产子宫下段“U”字型切口的应用

Evaluation of “U” incision of uterus low segment during caesarean section in second stage of labour.

目的 评价第二产程剖宫产子宫下段“U”字型切口的应用价值。方法 第二产程剖宫产 12 0例 ,随机分成两组 ,即子宫下段“U”字型切口组 6 2例 ,子宫下段横切口组 5 8例 ,比较两组患者在剖宫产术中及术后并发症发生情况及出血量。结果 子宫下段“U”字型切口组子宫切口撕裂 4例 ,占 6 .4 5 % ,而对照组子宫切口撕裂 12例 ,占 2 0 .6 9% ,两组比较差异显著 (P <0 .0 5 ) ,而平均出血量也有统计学意义 (P <0 .0 2 )。子宫下段“U”字型切口组术中出血量 <5 0 0ml者 5 9例 ,占 95 .16 % ,而横切口组为 4 2例 ,占 72 .4 1% ,两组比较有统计学意义 (P <0 .0 5 ) ,出血量≥ 5 0 0ml者前者为 3例 ,占 4 .86 % ,而后者为 16例 ,占 2 7.5 9% ,两组比较有显著性差异 (P <0 .0 5 )。结论第二产程剖宫产子宫下段“U”字型切口的应用可有效避免或减少子宫切口外延撕裂 ,减少术中出血 。

Objective: To evaluate of “U”incision of uterus low segment during Caesarean section in second stage of labour. Methods: 120 cases who were choiced at random were divided into two groups: study group that 62 cases were operated with “U”incision of uterus low segment during Caesarean section in second stage of labour, and control group that 58 patients were operated with crosscut incision of uterus low segment .Some complications such as uterus incision rupture and hemorrhea and so on were investigated between the two groups. Results: 4patients occurred uterus incision rupture while there were 12 cases in control group(P<0.05). On the other hand, the bleeding volume of study group was much more than that of control group(P<0.02). Conclusions The applications of “U”incision of uterus low segment during Caesarean section in second stage of labour could effectively avoid such complications as uterus incision rupture and decrease hemorrhage.
