摘要
目的分析MCS+血细胞分离机单一供者血小板(SDP)的采集质量与冲红原因。方法以MCS+采集196人份SDP,以KX-21血细胞分析仪并比较SDP成品中血小板(plt)含量、白细胞(WBC)污染量、红细胞(RBC)污染量、采前红细胞压积(Hct)及采前平均红细胞体积(MCV)等参数。结果196人份SDP成品中19人份SDP发生冲红现象,plt平均含量为(3.64±0.81)×1011袋,WBC、RBC污染量符合质量控制要求,分别占90.82%和33.16%。冲红组与未冲红组比较显示SDP成品中plt、采前Hct差异无显著性,而WBC污染、RBC污染及采前MCV差异有显著性(分别P<0.001,P<0.01,P<0.001)。结论MCS+采集的SDP其plt含量完全符合质量控制标准WBC严重污染几乎与冲红现象同步。采前MCV是导致冲红与否的关键,在其他指标符合要求的前提下,采前MCV≥88fL将可杜绝冲红现象的发生。
Objective To analyze the quality and study the cause of RBC spill-over in concentrating single donor platelet with MCS+ blood cell separator.Methods The 196 doses of SDP products collected with MCS+ and the parameters of platelet (plt) yield, white blood cell (WBC) contamination, red blood cell (RBC) contamination, pre-haematocrit (Hct) and pre-mean-cell-volume (MCV) counted with KX-21 blood cell analyzer were compared. Results Of all 196 doses of SDP, the RBC spill-over was 19 doses of SDP, the plt average yield was (3.64±0.81)×1011,the rates of WBC and RBC contamination within the quality control were 90.82% and 33.16% respectively. Comparing the group of RBC spill-over with the group of no RBC spill-over, it was found that the levels of plt yield and pre-Hct were variances insignificantly while the WBC contamination and the RBC contamination and the pre-MCV were different significantly(P<0.001,P<0.01,P<0.001,correspondingly).Conclusion The plt yield of SDP collected with MCS+ is fully met the criteria of quality control; the severe WBC contamination and RBC spill-over are almost happened synchronously. Pre-MCV is the key causing RBC spill-over, in the condition of other parameters meeting the requirement, the RBC spill-over could be prevented while the level of pre-MCV≥88fL.
