摘要
目的:比较急性早幼粒细胞白血病不同检测方法的特异性和敏感性。方法:采用细胞遗传学和巢式RT蛳PCR方法检测FAB法确诊的37例APL初治患者。结果:FAB方法误诊率13.5 %、准确度86.5 %,细胞遗传学方法漏诊率5.4 %、准确度94.6 %,巢式RT蛳PCR方法特异性100 %、灵敏度100 %、准确度100 %。结论:巢式RT蛳PCR检测PML蛳RARα融合基因特异性强、灵敏度高、快速,巢式RT蛳PCR结合FAB法和细胞遗传学方法可以提高APL诊断的准确度。
Objective: To compare the specificity and the sensitivity of different measurements of acute promyelocytic leukemia. Methods: 37 APL patients diagnosed by FAB method were measured by cytogenetics and nested RT- PCR. Results: The misdiagnosis ratio and accuracy of FAB method respectively were 8.1 % and 91.8 %. The pseudo- negative ratio and accuracy of cytogenetics were 5.4 % and 94.6 %. The specificity and sensitivity and accuracy of nested RT- PCR were 100 %. Conclusion: Nested RT- PCR have excessive specificity and sensitivity and speed to measure PML- RARα gene. The combination of FAB method and cytogenetics and nested RT- PCR can elevate the diagnostic accuracy of APL.
