脑诱发电位对酒依赖患者脑干功能及皮质认知功能的评估作用 被引量:7

摘要 目的:探讨酒依赖患者脑干听觉反应(audilorybraimstemresponse,ABR)及事件相关电位P300的特点,分析洒依赖患者认知功能损害情况。方法:收集到25名男性酒依赖患者以及29名男性正常人,洒依赖患者诊断符合中国精神病分类方案与诊标记第2版。使用丹麦仪器和click短声以及“听觉靶-非靶刺激序列”为诱发事件,完成ABR和P300检测。结果:酒依赖组ABR和P300波形变异明显,P300尤甚。与正常对照组相比,酒依赖组ABR绝对潜伏期波Ⅱ~Ⅵ波延迟(P<0.05或P<0.01),绝对波幅波Ⅱ、波Ⅲ、波Ⅳ和波Ⅴ降低(P<0.05或P<0.01)。酒依赖P300靶和非靶主成分潜伏期延迟,对照组P2和P3潜伏期分别为(171±22)ms和(302±17)ms,酒依赖组分别为(187±26)ms和(319±27)ms,差异有非常显著性意义P<0.01)波幅下降犤对照组P2和P3波幅分别(6.0±2.1)和(8.2±3.9)μV,酒依赖组分别为(3.9±2.3)μV和(3.5±3.1)μV犦。结论:提示长期大量饮酒既损害脑干功能,同时又损害皮质认知功能。 AIM:To explore the characteristics of auditory brainstem response(ABR)and event related potential(ERP)P300 in patients with alcoholism, and to analyze the condition of damage of their cognitive function. METHODS:Detections of ABR and P300 were performed in 25 male patients with alcoholism, who were in accordance with the diagnostic criteria of the second edition of Chinese Classification and Diagnostic Criteria of Mental Disorders(CCMD-2),and 29 normal male controls by using instrument of Denmark and Click,taking a standard oddball paradigm as the evoked event. RESULTS:Irregular wave forms of ABR and P300 were obvious in the alcoholic group,especially that of P300. Compared with the control group,ABR of the patient group showed the delayed absolute latencies of waves Ⅱ-Ⅵ)(P< 0.05 or P< 0.01) and the decreased amplitudes of wave Ⅱ, wave Ⅲ, wave Ⅳand wave Ⅴ(P< 0.05 or P< 0.01). The main components of both target P300 and non-target P300 in the alcoholic group presented prolonged latencies [P2 and P3 latencies were(171±22) ms and(302±17) ms in the control group,and(187±26) ms and(319±27) ms in alcoholic group, P< 0.01]and decreased amplitudes[P2 and P3 amplitudes were(6.0±2.1)and(8.2±3.9) μV in control group,and(3.9±2.3)and(3.5±3.1) μV in the alcoholic group, P< 0.05]. CONCLUSION:Long-term and large-amount alcohol drinking damages not only the cognitive function but also the brainstem function.
