正常成人腰椎X线影像计量学参数的测量与临床分析

Measurement of metrologlc Parameters and clinical analysis of lumbar vertebra X-rays in normal adults

作者拍摄了７０名正常中国成人腰椎Ｘ线平片，男女各一半，１０岁为一年龄组，每组７人。用ＭＯＰ－Ｖｉｄｅｏｐｌａｎ图像仪描记，测得１０项包括椎体宽度、长度、高度、面积等二维参数及计算的三维体积参数。数据表明，男性各椎体的理想圆柱体积及各项参数的均值与年龄显著相关，而女性则无相关。最后，我们尝试性解释男女这种差异的可能机理。女性因为绝经期后骨量的迅速丢失，其椎体体积的相对恒定可能是妇女免于压缩性骨折的一种保护机制。这些参数的建立，为诊断Ｘ线平片目视不易诊断的轻微骨折提供了一种方法。另外，男性椎管狭窄发病率高于女性的原因可能由于男性椎体随年龄增加而不断增大所致。

Anterior-posterior and lateral x-ray films of lumber vertebrae were taken from 70 normal Chinese, including 35 males and 35 females. By age, each group was subdivided into 5 subgroups, 7 for each. With MOP-VIDEOPLAN image analysis system,both AP and L vertebral films were analyzed by tracing their image. There were 10 parameters available after the measurement for the quantitative analysis. They were area, perimeter, maximum diameter, width and height for the two dimensional measurements and vertebral volume for three dimensional measurements. The data suggest that the ideal column volume of each vertebra has significant correlation with age in the male group,but has not in the female group(P>0. 05). In conclusion,an attempt was made to explain the significant difference between sexes,It may be true that the volume of lumber vertebrae do not increase after menopause. In contrast,the vertebral volume of males may expand continually in whole life span because their bone loss is relativiy less,We would like to suggest that the constant volume of lumbar vertebrae might be a protective mechanism for women to protect themselves from compression fracture. These parameters provide a method for the diagnosis of mild fracture that is uneasy to be diagnosed in X-ray films. Besides,the prevalence of vertebral narrowing is higher in males than in females,which is due probably to the continuous expansion of vertebrae with aging.
