摘要
结缔组织病人由于代谢失常或服用激素等原因常发生骨质疏松,研究结缔组织病人骨密度及血清钙水平,对了解该病的钙磷代谢状况和骨质疏松程度,估计辅助治疗效果及预防激素治疗副作用提供一定的临床依据。本文观察我院住院的绝经前结缔组织病人28例,和相应年龄正常女性29人,用SPA法(Singlephotonabsorptiometry)检测桡、尺骨密度,用络合指示剂法测定血清钙。结果表明:病人组与正常组比较,骨密度和血清钙水平均降低,两者均呈显著性差异(P<0.05)。病人组桡、尺骨骨密度及血清钙浓度均降低,且两者降低水平呈正相关,r=0.74。随年龄增长,桡、尺骨密度及血清钙水平均降低,病人组下降更突出,但无显著性差异。测定骨密度和血清钙水平,可简便特异性地评估结缔组织病人骨矿物质丢失及骨质疏松状况,对估计补钙保骨的辅助治疗效果有一定临床意义。
Patients with connective tissue diseases (CTD) are often complicated with osteoporosis on account of disturbance of body metabolism or administration of steroid hormones. So the determination of bone density (BD) and plasma calcium are clinically important to the evaluation of the state of calcium and phosphorous metabolism and to help the evaluation of the efficacy of treatment.We adopted single photon absorptiometry (SPA) to measure the bone density (BD) of ulna and radius in 28 cases of premenopausal CTD and determined plasma calcium by complexiometry simultaneously. The control group was composed of 29 normal premenopausal students or workers and their relatives. The results are as follows:(1) The average values of either BD or plasma calcium in CTD group are lower than those in the control group,their difference being significant statistically (P<0. 05).(2) The BD and plasma calcium,no matter in the CTD group or the control females,tend to decrease with increasing age.(3) The differences between BD and plasma calcium in 21~40 or 41~60 year-old premenopausal groups of both the CTD group and the control females are not significant (P>0. 05).(4) The BD and plasma calcium levels decrease with increasing age,particularly in CTD patients and show positive correlation (r=0. 74).The measurement of BD by SPA is a simple,specific,accurate and nontraumatic method for early diagnosis of osteoporosis; together with determination of plasma calcium level,it might be an important means to estimate the efficacy of therapy. So it has sufficient clinical significance.
