小儿溃疡病急性穿孔的诊治体会(附10例报告)

Experience of Diagnosis and Treatment of Acute Perforation of Ulcer in Children

目的探讨和总结小儿溃疡病急性穿孔的临床特点和手术方法。方法对我院1990年～2001年间经手术治疗的小儿溃疡病急性穿孔进行回顾性研究,总结小儿溃疡病急性穿孔的临床表现和检查方法,并对手术方法进行评价。结果10例均经手术治愈,无死亡,随访半年～2年,无溃疡病复发。结论小儿消化性溃疡合并急性穿孔的诊断不能完全依赖于病史,要综合分析,一旦确诊应紧急手术,手术方法以大网膜覆盖穿孔缝合术为合理。

Objective To discuss and summarize the diagnosis and treatment of acute perforation of gastroduodenal ulcer in children. Methods Children with acute perforation of gastroduodenal ulcer underwent operative therapy in our hospital between 1990 and 2001 were studied retrospectively. The clinical manifestation and examination method,and the operation techniqe was appraised. Results 10 cases underwent operation were cured and no died. There was no relapse of ulcer during 0.5～2 years' followed-up. Conclusions Diagnosis of acute perforation of gastroduodenal ulcer in children not only depends case history,but needs comprehensive analysis.The operation should be performed,as long as the acute peptic ulce perforation was definitely diagnosed. The reasonable operative procedure is suturation of perforation covered with omentum majus .
