摘要
目的 :分析胃癌组织P 糖蛋白 (P Glycoprotein ,P gp)和多药耐药相关蛋白 1(MRP1)的表达及其与肿瘤化疗敏感性和化疗方案选择的关系。方法 :将 4 0例胃癌标本 (标本组 )行肿瘤细胞化疗敏感性测定 (H3 TDR法 ) ,并分析其与肿瘤P gp、MRP1表达的关系 ;另随访P gp、MRPI阳性表达率相近的 30例 (化疗组 )术后化疗的胃癌患者 ,了解化疗效果与P gp、MRP1表达的关系。结果 :4 0例胃癌标本P gp表达阳性率 4 7.5 % (19例 ) ,阴性率 5 2 .5 % (2 1例 ) ;MRPI阳性率 2 2 .5 % (9例 ) ,阴性率 77.5 % (31例 )。化疗组 30例 ,P gp阳性率 4 0 % (12例 ) ,阴性率 6 0 % (18例 ) ;MRPI阳性率 2 3.3% (17例 ) ,阴性率76 .7% (2 3例 )。化疗药物对胃癌细胞均有一定的抑制生长作用。MDR1表达会影响肿瘤细胞对ADM和VP 16的敏感性 ;MRP1表达会影响肿瘤细胞对ADM和 5 Fu的敏感性。随访的 30例胃癌术后化疗患者 ,死亡 11例。P gp和MRP1均阴性者存活期最长 ,其次为其中一个阳性表达 ,P gp和MRP1均阳性者无存活。结论 :P gp和MRP1表达对化疗的效果有重要影响 。
Objective: To study the expression of P-glycoprotein (P-gp) and multiple resistance protein-1 (MRP-1) in gastric cancer and its relation with tumor chemosensitivity. Methods: The chemosensitivity to 5 anti-cancer drugs in 40 samples from gastric cancer patients was studied through the method of H 3-TDR and analyzed its relation with the expression of P-gp and MRP-1. Then, 30 gastric cancer patients with postoperative chemotherapy were analyzed for their survival time and the expression of P-gp and MRP-1. Results: 32 cases in two groups showed P-gp expression; 8 cases showed MRP1 expression; 9 cases were both positive and 21 cases were both negative.Anti-cancer drugs could inhibit the growth of tumor. P-gp could change the sensitivity of gastric cancer cells to ADM and VP-16; MRP-1 could change the sensitivity of gastric cancer cells to ADM and 5-Fu. In the follow-up group, 11 out of 30 patients died. The patients with negative expression of both P-gp and MRP-1 had the longest survival time. Patients with either P-gp or MRP-1 positive expression had longer survival time. However,patients with P-gp and MRP-1 positive expression had the shortest survival time.Conclusion:The expressions of P-gp and MRP1 play important role in the chemotherapy. Detection of them can help us to select anti -cancer drugs.
