放置食管记忆支架引发的并发症及防治措施

Prevention and cure to the complication of esophageal memory allay supporter

目的 探讨放置食管记忆合金支架的并发症及防治措施。方法 对 5 8例食管良、恶性狭窄及食管气管瘘患者通过放射、胃镜及手术方式放置MTN型记忆镍钛合金食管支架 ,观察并发症 ,采取相应措施进行防治。结果 对所有患者随访 3- 36个月 ,发现各种并发症有疼痛不适、异物感者 5 8例 ,恶心、呕吐 2 0例 ,出血 18例 ,胃食管反流 10例 ,支架移位、错位、脱落 8例 ,再狭窄 3例 ,压迫气管 1例 ,大出血 1例。结论 记忆支架扩张食管满意 ,明显改善患者生活质量 ,同时近远期并发症也不少。对可能或出现并发症者采取相应措施进行预防和治疗 ,可达到预期的效果。

Objective To discuss prevention and cure measures to the complication of esophageal memory allay supporter.Methods 58 cases of esophageal narrow and fistula of esophagus-trachea were carried to put MIN model memory nickel-titan allay supporter by radiation, gastroscope and operations methods, we observed complications of esophageal memory allay supporter and took corresponding measures to prevent it.Results All patients were followed-up 3-36 months.There were many kinds of complications, such as pain, 58 cases of foreign body sensation, 20 cases feeling to sick to vomit, 18 cases to bleed, 10 cases of gastroesophageal reflux, 8 cases of esophageal supporter being taken to displacement, malposition and abscission, 3 cases to restenosis, 1 case to press hard to trachea and 1 case to massive hemorrhage. Conclusion Memory support can expand esophagus in a good satisfaction.It can not only improve patients quality of life, but also bring some recent and long-term complications as well.It could achieve expected effects if we had taken corresponding steps to prevent and cure possible complications.
