胃食管返流对稳定期COPD病人症状的影响

Symptom Effect about Gastroesophageal Reflux to Patients of COPD in the Stationary Phase

目的 探讨胃食管返流 (GER)对稳定期COPD病人症状的影响 .方法 对 36例有症状的稳定期COPD病人和 2 0例正常对照组进行 2 4小时食管pH监测、症状相关性分析和抗返流治疗验证 .结果 5 2 .7%病人 (19/36 )De meester总积分≥ 14 .72 .4 7.2 % (17/36 )病人咳嗽与返流有较高相关性 (SAP≥ 75 % ) ,81% (13/16 )病人返流症状与返流有较高相关性 (SAP≥ 75 % ) .抗返流治疗后 ,治疗组 10 0 %病人反胃嗳气、胸骨后烧灼感消失 ,6 6 .7% (8/12 )病人咳嗽消失 ,83.3% (10 /12 )病人咳嗽好转 ,与对照组比较有显著差异 (p <0 .0 5 ) .结论 ①有症状的稳定期COPD病人GER发生率较高 .②GER是导致稳定期COPD病人症状加重的重要因素之一 .③抗返流治疗可显著减少稳定期COPD病人部分症状 ,从而提高病人的生活质量 。

Objective To explore symptom effect about gastroesophageal reflux(GER) to patients of COPD in the stationary phase. Methods 36 COPD patients in the stationary phase and 20 common control group were enrolled in the study. 24-hour ambulatory esophageal pH monitoring was performed and symptoms,such as cough, chest tightness and pyrosis were recorded. Patients with diagnosed GER were divided into two groups with one receiving anti-reflux treatment. Results DeMeester score ≥14.72 was found in 52.7%(19/36)patients. After anti-reflux therapy,symptoms such as cough , reflux symptom alleviated significantly compared with that of control group(p<0.01). Conclusions ①Incidence of GER was 52.7% in this groups of patients of COPD in the stationary phase. ②24 hour esophageal pH monitoring was of great value in diagnosing GER in those patients. ③Anti-reflux therapy ameliorated symptoms induced by GER in those patients.
