传统与新型抗抑郁药对抑郁症病人疗效及生活质量影响的双盲对照研究

The efficacy and life quality comparison of typical and atypical antidepressant for depression

目的了解传统与新型抗抑郁药治疗抑郁症病人的疗效及对生活质量的影响。方法将40 0例抑郁症病人按门诊病历号分为两组 ,奇数为传统抗抑郁药组 (服用多虑平、丙咪嗪、阿米替林 ) ,偶数为新型抗抑郁药组 (服用美抒玉 ,盐酸氟西汀 ,赛乐特 ) ,治疗时间为 6周 ,分别于疗前和疗后 2、4、6周使用汉密顿抑郁量表 (HAMD) ,临床总体印象量表 (CGI) ,副反应量表 (TESS)进行评分。采用双盲对照研究 ,生活质量综合评定问卷 (GQOLI)分析病人生活质量。结果两组有效率分别为 82 .5 %和 87% (P >0 .0 5 ) ,副反应两组在植物神经、心血管以及神经系统方面差异有显著性 (P <0 .0 5 )新型抗抑郁药组的躯体健康维度 ,心理健康维度及社会功能维度得分明显高于传统抗抑郁药 ,均有显著性差异 (P <0 .0 1)。结论新型抗抑郁药对抑郁症病人的生活质量的影响优于传统抗抑郁药 ,并且副反应小 ,依从性好。

Objective TO compare efficacy and life quality of typical and atypical antidepressant in depressive patients. MethodsFour hundred out- or in-patients according to the number of cases were randomly assigned into two groups. The patients with odd number treated with typical antidepressant as doxepin, imipramine, amitriptyline, patients with even number treated with atypical agents including trazodone, fluoxetine, paroxetine. The course was six weeks. All patients were evaluated with HAMD, CGI and TESS before the treatment and at the weekend of 2,4,6. According to the method of double-blind study, the life quality was analyzed with GQOLI. Result The efficacy of two groups were respectively 82.5% and 87% and there were no significant difference. But the difference of side effects on autonomic nervous system, nervous system and vascular system were obvious. The atypical antidepressant's healthy dimensionalities of body, mental and social function were superior to that of typical antidepressant. Conclusion Effect of atypical depressant to patients' life quality was superior to that of typical depressant, side effect was small and the obedience was good.
