^(18)F-FDGhPET/CT鉴别肺部肿块

The Value of Differential Diagnosis for Pulmonary Lesion with ^(18)F-FDG Dual-Head Gamma Camera Coincidence Imaging

摘要 目的 评价18F 氟脱氧葡萄糖双探头符合线路显像对肺部占位病变鉴别诊断的意义。方法 18F 氟脱氧葡萄糖双探头符合线路显像检测 4 7例 ,病理证实恶性肿瘤 37例 ,良性病变 10例。用目测法及半定量法测量病灶的靶 /非靶比值 ,并且比较良、恶性肿瘤组之间病灶直径和靶 /非靶的区别 ,以及病灶直径与靶 /非靶之间的关系。结果 ①18F 氟脱氧葡萄糖双探头符合线路显像阳性 4 0例 ,阴性 7例 ,灵敏度 97% ,特异性6 0 % ,准确率 89% ,阳性预测率 90 % ,阴性预测率 85 %。② 4 7例中有病灶 6 0个 ,其中恶性病灶 4 6个 ,直径1 0~ 8 1cm ,平均 (3 14± 1 4 6 )cm ;良性病灶 14个 ,直径 0 8~ 7 8cm ,平均 (3 2 1± 2 4 8)cm(P >0 0 5 )。4 6个恶性病灶和 14个良性病灶的靶 /非靶比值分别是 1 5 7~ 2 0 75 (平均 7 6 4± 4 92 )和 1 85~ 15 4 (平均4 2 8± 3 2 4 ) (P <0 0 0 5 )。③本组病灶直径与靶 /非靶比值之间有正相关关系 (r =0 4 5 ,P <0 0 5 )。结论 18F 氟脱氧葡萄糖双探头符合线路显像对鉴别诊断肺部良。 Objective To assess the differential diagnostic value for pulmonary lesion with 18F-FDG dual-head gamma camera coincidence imaging ( 18F-FDGhPET/CT). MethodsForty-seven cases were studdied by 18F-FDG hPET/CT. The studies used visual and semiquantitative methods for differentiation of the malignant and benign lesions. We also compared the differences of the diameter and target/non-target(T/NT) ratio of lesions as well as the correlation between the diameter and T/NT ratio between the malignant and benign patients.Results ①Thirty-seven cases had malignant and ten cases had benign pulmonary lesions proven by pathology. Forty cases were positive and seventy cases were negative in FDG imaging. The FDG study showed 97% sensitivity, 60% specificity, 89% accuracy, 90% positive prognostic value and 85% negative prognostic value. ② Among 47 cases, there were 60 lesions including 46 malignant lesions (the size 1.0~8.1 cm, mean 3.14 cm±1.46 cm) and 14 benign lesions (the size 0.8~7.8 cm, mean 3.21 cm±2.48 cm)(P>0.05). The T/NT ratios were 1.57~20.75(7.64±4.92)for malignant lesions and 1.85~15.4(4.28~3.24) for benign lesions (P <0.005). ③ There was positive correlation between the size and the T/NT ratio of the lesions (r=0.45, P<0.05).Conclusion FDG-hPET/CT has significant value for differential diagnosis of pulmonary lesions.
