38例严重缺损牙的保存治疗

目的 利用综合治疗 ,对残根、残冠、纵折牙等严重缺损牙进行保存治疗并分析其疗效。方法 共38例 39颗严重缺损牙 ,采取综合治疗方法 ,包括结扎固定、根管治疗、牙体半切术、后磨牙分根术、根管钉固位、桩钉 (核 )固位等 ,辅以烤瓷冠修复 ,恢复患牙正常形态与功能。治疗完成后 6、12、2 4个月检查患牙情况。结果 6个月时 39颗患牙中有 1颗全冠因粘结不良脱落 ,12个月时复查 2 9颗患牙中有 2颗出现牙周损伤。经过治疗后 ,它们都恢复正常功能。 2 4个月时复查 18颗患牙功能均正常。治疗有效率达 93 36 %以上。结论 严重缺损牙的综合治疗效果明显 。

Objective To analyze the effect of preserving severely damaged teeth such as stubbed tooth root、crown and vertically fractured tooth through compositive therapy. Methods 39 teeth in 38 patients were treated by a series of methods including ligating、RCT、tooth semi-extraction、tooth-root-resection、root-pin retaintion、post retaintion and ceramic crown restoration. After 6、12、24 months of therapy, patients were recalled and teeth were checked. Results One of the 39 teeth had its crown fallen because of bad-bonding at 6th month and two of the 29 teeth suffered periodontal damage at 12th month. All of them regained normal function after treatment. All of the 18 teeth functioned well after 24 months. The success rate of compositive therapy was above 93.1%.Conclusion Compositive therapy of severely damaged teeth had a perfect effect and a promising clinical value.
