单侧经椎间孔后路椎间融合与内固定治疗腰椎滑脱

Unilateral Transforaminal Posterior Lumbar Interbody Fusion and Internal Fixation for Lumbar Spondylolisthesis

目的 探讨单侧经椎间孔后路椎间融合与内固定治疗腰椎滑脱症的临床效果。方法 应用单侧经椎间孔后路椎间融合与 TENOR脊柱内固定系统 ,以提高复位效果及植骨愈合率 ,维持脊柱的持久稳定。结果 全部病例经3～ 16个月 ,平均 9个月的随访 ,2～ 3个月可见骨痂生长 ,6个月达椎间融合 ,钛网椎间融合器无移位、无下沉 ,内固定系统无松动、断裂 ,患者症状与体征恢复满意。结论 TENOR内固定系统直接复位滑椎 ,并获得即刻稳定 ;提高融合率 ,钛网融合器对椎体的前中柱有支撑作用 ,恢复椎间高度 ,维持腰椎的生理弧度 ,促进植骨融合。

s:Objective To investigate the results of unilateral transforaminal posterior lumbar interbody fusion and internal fixation for lumbar slippage. Methods All cases were followed up 3～16 months with average of 9 months. Unilateral transforaminal posterior lumbar interbody fusion in combination with TENOR internal fixation system was used to improve the results of reduction and bone grafting and to maintain long term spinal stability. Results There was radiographic evidence of bony callus after operation 2～3 months and interbody fusion after operation 6 months. There was no evidence of titanium mesh migration and subsidence,and no loosening and breaking of TENOR system. Patients satisfied with improvement of symptoms. Conclusions TENOR internal fixation system directly reduces lumbar slippage and attains short term stability,while titanium mesh fusioner strengthen its effect,resume spinal height,maintain lumbar physiological curvature,promote grafted bone fusion.
