视听整合连续测试对注意缺陷多动障碍的评估作用(英文) 被引量:2

摘要 背景:对于儿童注意缺陷多动障碍(attentiondeficiencyandhyperactivitydisorder,ADHD)的诊断和评估,多年来一直采用主观的量表评定法,如何用客观的检查手段进行更正确、合理的诊断,是近年来探索的一个课题。目的:探讨视听整合连续测试对在ADHD儿童中的评估价值,以及客观、易量化的评估ADHD的方法。设计:横断面研究。地点和对象:对2003-07/10在无锡市儿童医院心理行为门诊初诊的瑞文智商筛查正常,症状持续半年以上,疑似ADHD的全部儿童157例。干预:采用视听整合连续测试进行评估,分析被诊断为ADHD儿童听、视觉注意能力的差异。主要观察指标:视听整合连续测试所得18个商数指标。结果:反应控制商数与注意力商数结果提示:注意力缺陷的发病率高于自控力差、冲动的发病率,差异有显著性意义(χ2=37.74,P<0.01);在反应控制商数中,听觉的控制能力比视觉的控制能力差,差异有显著性意义(χ2=14.68,P<0.01);注意力商数中的听、视觉对比,差异无显著性意义(χ2=3.56,P>0.05)。视听整合连续测试诊断为ADHD的96例儿童听觉的一致性(68例)、警惕性(66例)、注意力商数(50例)<85分值的比例较高;视觉的警惕性(58例)、速度商数(61例)<85分值的比例较高,差异均有显著性意义(χ2=6.75~22.81,P均<0.01)。结论:视听整合连续测试能? BACKGROUND:Subjective checklist test was applied in the diagnosis and assessme nt of attention deficiency and hyperactivity disorder(ADHD)in children over the past years.How to make a more accurate and reasonable diagnosis has been much e xplored recently. OBJECTIVE:To explore the value of audiovisual integration continuous test in c hild ADHD and more objective and quantitative method for ADHD. DESIGN:Cross-sectional study. SETTING and PARTICIPANTS:A total of 157 children with normal intelligence quot ient and symptom duration over half a year were suspected as ADHD in the Psychol ogy and Behavior Outpatient Department of Wuxi Children's Hospital from July 200 3 to October 2003. INTERVENTIONS:Differences in auditory and visual attention ability of the chil dren were assessed using audiovisual integration continuous test. MAIN OUTCOME MEASURES:Eighteen quotient indexes in audiovisual integration con tinuous test. RESULTS:Results of reaction-control and attention quotient showed that£othe m orbidity of attention deficiency was higher than that of poor control ability an d impulse,with significant differences£¨|?2=37.74,P< 0.01£?.In reactiona2control quotient,control ability of auditory sense was poorer than that of visual sense with significant differences£¨|?2=14.68,P< 0.01£?.There was no significant diff erence in attention quotient between auditory and visual senses(|?2=3.56,P >0.05 £?.Proportion of auditory consistency(68 patients),vigilance(66 patients),and at tention quotient(50 patients) < 85 in 96 patients of ADHD diagnosed by audiovisu al integration continuous test was high;proportion of visual consistency(58 pati ents),speed quotient(61 patients)< 85 was high.Both had significant differences £¨|?2=6.75£-22.81,P< 0.01£?. a$CONCLUSION£oAudiovisual integration continuous test can effectively and compre hensively present the distribution and extent of attention and control ability d eficiency in ADHD children£which leads to accurate intervention projects and im provement of diagnosis and treatment.
