经膀胱与经尿道前列腺切除治疗高龄及高危前列腺增生62例分析

Combined transvesical prostatectomy and transurethral resection of prostate for the treatment of benign prostate hyperplasia in patients with high risk and aging

目的 探讨治疗高龄及高危前列腺增生的最佳手术方法。方法 对 6 2例高龄高危前列腺增生患者联合应用经膀胱前列腺切除术与经尿道前列腺切除术进行治疗。结果 6 0例 1次电切均成功 ;1例因术后出血行二次电凝止血 ,1例分两次手术。手术操作时间平均 4 2min ,平均失血量 90ml,平均切割前列腺组织 18g ,未输血 ,无前列腺切除术综合征发生。无一例发生心、脑、肺、肾等合并症加重。结论 联合应用经膀胱前列腺切除术与经尿道前列腺切除术 ,可以兼容二者长处 ,尤其适合于高龄高危前列腺增生患者。

Objective To explore a safe and effective way to treat the benign prostate hyperplasia(BPH) patients with high risk and old age.Methods Combined transvesical prostatectomy(TVP) and transurethral resection of the prostate(TURP)was undertaken for 62 BPH patients with high risk and advancd age.Results Satisfactory result was achieved in 60 cases in one TVP.Only 1 patient needed second surgery because of postoperative bleeding.The operation was divided into twice in 1 case.The average duration of the procedure was 42 min,the average blood loss 90ml and the resected prostate weigh 18 g.No blood transfusion was adopted and no transurethral resection syndrome occurred.None had severe complications.Conclusion Combined TVP and TURP resection of prostate is an effective way to treat BPH patients with high risk and advanced age.
