苦参素注射液治疗慢性乙型肝炎近期疗效观察

Therapeutic effect of oxymarrine injection on chronic hepatitis B

目的 观察苦参素注射液治疗慢性乙型肝炎的疗效。方法 5 8例慢性乙型肝炎患者随机分为治疗组及对照组 ,治疗组使用苦参素注射液 ,对照组使用复方益肝灵片等药物 ,观察 2组治疗后丙氨酸氨基转移酶 (ALT)复常率及乙型肝炎病毒e抗原 (HBeAg)、乙型肝炎病毒脱氧核糖核酸 (HBV DNA)阴转率。治疗组中 19例患者治疗前后做肝组织活检 ,采用Knodell计分法观察治疗后肝脏组织学变化。结果 治疗 3个月后治疗组ALT复常率、HBeAg、HBV DNA阴转率分别是 5 3.3%、4 0 %、4 6 .7% ;对照组分别是 2 5 %、7.14 %、7.14 %。 2组比较有显著性差异 (P <0 .0 5 ,P <0 .0 1,P <0 .0 1)。治疗组 19例患者治疗后肝组织学中肝细胞坏死、小叶内炎症及纤维化明显减轻 (P <0 .0 5 ) ,而门静脉炎症改善不明显 (P >0 .0 5 )。结论 苦参素注射液治疗慢性乙型肝炎在肝功能ALT复常率、肝脏组织学病理改善以及HBeAg、HBV

Objective To observe the therapeutic effect of oxymarrine injection on chronic hepatitis B. Methods 58 patients with chronic hepatitis B were randomly divided into two groups. 30 patients in treatment group were treated by oxymarrine injection;28 patients in control group were treated by compound Yiganling tablets and routine western medicines, with a treatment course of three months for both groups. The recuperation rate of ALT and negative-conversion rate of HbeAg and HBV-DNA were measured after treatment. The biopsy of liver tissue in 19 patients of treatment group was performed and observed by Knodell score method before and after treatment. Results ZAfter 3-month's treatment the recuperation rate of ALT and negative-conversion rate of HBeAg and HBV-DNA in treatment group were 53.3%,40.0% and 46.7% respectively, which were 25.0%,7.1% and 7.1% in control group respectively. There were significant differences between two groups (P<0.05,P<0.01,P<0.01).After treatment the pathological changes in liver tissue of 19 patients in treatment group were obviously relieved (P<0.05),however, the improvement of portal inflammation was not obvious (P>0.05). Conclusion The oxymarrine injection has advantages in the treatment of chronic hepatitis B,such as in the improvement of pathological changes of liver tissue, the recuperation rate of ALT and negative-conversion rate of HBeAg and HBV-DNA.
