迟发性外伤性颅内血肿68例临床分析

Clinical Analysis of 68 Cases with Delayed Traumatic Intracranial Hematoma

目的 探讨迟发性外伤性颅内血肿 (DTICH)的早期CT征象、临床表现及早期诊治。方法 对 1 992年1月～ 2 0 0 3年 1月期间的 68例DTICH进行回顾性分析总结。结果 本组 68例患者 ,手术治疗 47例 ,保守治疗 2 1例 ,痊愈 43例 ,中残 4例 ,植物生存 1例 ,死亡 1 7例 ,死亡率 2 5 % ,3例自动转院。结论 对脑外伤患者应考虑有DTICH的发生 ,应尽早复查CT,早诊早治可改善预后。

Objective To study the earlier CT findings, clinical symptom, earlier diagnosis and treatment of delayed traumatic intracranial hematoma (DTICH). Methods Retrospective summarization was done for 68 cases with DTICH from January 1992 to January 2003. Result Among the 68 cases, 47 cases received operative treatment, 21 cases received conservative treatment, 43 cases were cured and 17 cases died (the mortality was 25%), 4 cases led to moderate disability and one cases had accepted vegetal survival and 3 cases leaved hospital midway. Conclusion It showed that for patient with head injury DTICH should be considered. Repeating CT scan as early as possible will always be favorable. Earlier diagnosis and treatment may improve prognosis.
