脊椎椎板复位成形术在颈椎病变手术中的应用

Laminoplasty of Cervical Intraspinal Diseases with A Replacement and Reconstruct of Laminae

目的探讨脊椎椎板复位成形术在颈椎椎管后部椎板切开入路手术中的应用价值。方法在颈椎病变椎板切开入路手术中将取下的椎板复位,以此保障脊柱正常结构的完整性、避免脊柱的术后变形。我科从2001年10月-2002年10月在104例颈椎病变椎板切开后部入路手术中有82例(79％)的病例使用了椎板复位成形术。结果所有行椎板复位成形术的手术均获成功,术后复查x光平片、计算机体层摄影术或磁共振成像未发现椎板滑脱及固定丝线脱落及断裂的表现,椎板成形完好脊柱序列正常,未发现异常并发症,病人术后颈部运动正常。结论颈椎椎板复,立成形术方法简单可靠,经济实用,在颈椎椎板切开入路手术中运用效果好,实用价值高,是一种实用的、值得推广的、有价值的手术方式。

Objective To study the effective of laminoplasty of cervical intraspinal diseases with a replacement and reconstruct of laminae. Methods The laminae were removed using a micro bone drill in the laminectomy at any part of the cervical spinal column which need operated, then the original laminae were replaced and reconstructed during operation. This kind of operations can keep the integrality of spinal column with nonnal skeletal structure and its serial and avoid spinal deformity after operation. 82 of 104 cases(79% ) of laminoplasty of cervical intraspinal diseases made use of the laminoplasty in our department from Oct.2001 to Oct.2002. Results All cases got good outcome and no complications by approved with X-ray, CT and MR. Conclusion The laminoplasty is a simple, useful, effective operation for the need laminectomy of cervical intraspinal diseases.
