广东省妇幼保健院18年剖宫产率及适应证变化分析

The Analysis of Cesarean Section Rates in 18 Years in the Women and Children's Hospital of Guangdong Province

目的:探讨剖宫产率升高的原因及适应证的变化。方法:对18年剖宫产病例进行回顾性分析。结果:剖宫产的适应证发生了明显的变化。新生儿窒息率有明显下降,围产儿死亡率无明显改变。结论:剖宫产率的升高在一定程度上降低了新生儿窒息率,但围产儿死亡率的下降依赖于孕期完善的保健制度及分娩期严密监护,剖宫产率的增高并不能降低围产儿死亡率。

Objective;To investigate the changing trend of cesarean section rates in 18 years in our hospital. Methods; Samples taken from 4 months ( March, June, September, December) of one in every 3 years during 18 years peroid (from 1985 to 2002) were analysed and a total of 1001 cases were collected. Results; Cesarean section rates increased since the mid - 1990's and particularly since 1997. The cause of this trend may be related to the following reasons (1) Widely utilization of B ultrasound and fetal monitoring methods; (2) Improvements of skill and safety of cesarean section; (3) Social factors; (4) Indications changes of cesarean section. Conclusion; The decrease of perinatal mortality rate was not relevant to cesarean section rate, but dependent more on qualified obstetric care.
