923例青光眼住院患者的临床资料分析

Clinical data analysis of 923 glaucomatous patients

目的 了解 2 0 0 0年以来我院青光眼住院患者的患病类型、构成比、男女患者比例 ,术前平均视力和单、双眼盲目率 ,治疗方式以及术中抗代谢药物的应用情况等。方法 回顾性分析 2 0 0 0年以来 92 3例青光眼住院患者的各项临床资料。结果 原发性闭角型青光眼占 6 0 .1 3% ,其次为继发性青光眼 ,占 1 8.85 %。原发性青光眼中 ,男女之比为 1∶ 1 .1 0 ;其中急性闭角型青光眼男女之比为 1∶ 2 .4 8。在急性闭角型青光眼中 ,大于 5 0岁发病的患者占 91 .80 %。患病眼的平均病史为 (1 .97± 3.89)年 ,术前平均视力为 0 .2 6± 0 .2 37,平均 C/ D比值为 0 .6 9±0 .2 85 ;单眼盲目率为 2 1 .94 % ,双眼盲目率为 1 0 .2 1 %。手术治疗占 95 .0 2 % ,药物治疗占 4 .0 1 %。小梁切除术为最常用的抗青光眼术式 ,占 6 8.89% ;其次为 Nd:YAG激光周边虹膜切除术 ,占 9.4 3% ;再次为周边虹膜切除术 ,占7.88%。术中抗代谢药物 (丝裂霉素 C)的使用率为 86 .6 7% ,平均放置时间为 (1 .35± 1 .0 8)分钟。结论 原发性闭角型青光眼仍为常见类型的青光眼 ;急性闭角型青光眼以女性多见 ;左眼盲目率大于右眼盲目率 ;

Objective To find out what is the major type,the rate of component,the proportion of gender,the average preoperational vision and the rate of inducedblindness in one eye or both eyes,the treatment method,and the rate of application of antimetabolic drug combined with trabeculectomy,etc,in inpatients with glaucoma in our hospital since 2000.Methods Clinical data of 923 inpatients with glaucoma were analyzed retrospectively.Results The rate of component in primary angle closure glaucoma is 60.13%;the following is secondary glaucoma (18.85%).The total proportion of gender is 1∶1.10 in primary glaucoma,about 91.80% patients are over 50 in acute angle closure glaucoma.The average illness history is (1.97±3.89) years.The average vision of preoperational is 0.26±0.237,the average ratio of C/D is 0.69±0.285,the rate of induced blindness in one eye or bothe eyes is 21.94% and 10.21%,respectively.About 877/923 cases (95.02%) were chosen surgery.Trabeculectomy is the major anti glaucoma surgery (68.89%).Mitomycin C (MMC) is the common antimetabolic drug during the surgery.The rate of application of MMC combined with trabeculectomy is 86.67%,and the average placement time is (1.35±1.08) minute.Conclusion Primary angle closure glaucoma is still the most common type in clinic.Femle is familiar in acute angle closure glaucoma.The rate of induced blindness in left eye is more than in right eyes.Trabeculectomy is the major anti glaucoma surgery.
