摘要
目的 探讨老年大鼠肺部感染后血气、血流动力学改变的发生机制。 方法 4 0只雄性老年大鼠 ,随机分为 3个不同时间处死组及对照组。观察血流动力学参数、血气分析指标、血浆肿瘤坏死因子 α(TNF α)水平、心肺组织病理及TNF α免疫组化。 4 0只成年雄性大鼠作对照。 结果 大鼠肺部感染后动脉血氧分压 (PaO2 )、血氧饱和度 (SaO2 )降低 ,血浆TNF α增高 (P <0 0 5 ) ,心肌及肺组织TNF α表达增多。心率、等容收缩期左室内压力最大上升速率 (+dp/dtmax)、等容舒张期左室内压力最大下降速率 (-dp/dtmax)均降低 ;左心室舒张末期压 (LVEDP)升高、等容舒张期左室内压力下降的时间常数 (T值 )延长 ,肺部感染后 4 8h、72h ,老年组LVEDP〔(6 5± 0 7)、(8 9± 1 0 )mmHg〕,均高于对照组 (4 4± 1 3)和非老年组〔(6 4± 1 1)、(4 2± 1 2 )mmHg〕 ,均为P <0 0 5。 结论 老年大鼠肺部感染后血气及血流动力学改变持续时间长 ,可能有不同的分子网络机制。
Objective To study the mechanism of changes in blood gas and hemodynamics in aged rats with pneumonia. Methods Forty aged male Sprague-Dawley(SD) rats were randomly divided into three experimental groups and the control group. They were contrasted by 40 non-aged male SD rats. Hemodynamic parameters and blood gas were measured, and morphological changes of heart and lung were observed. Plasma levels of tumor necrosis factor-α (TNF-α) was examined by immunohistochemical method. Results PaO 2 and SaO 2 were lower in rats with pneumonia than those in the control rats(P<0.05).Plasma TNF-α concentration was enhanced in rats with pneumonia (P<0.05) and the expression of TNF-α was increased in myocardial and pneumonic tissues. LVEDP and T values were increased. The values of LVEDP of 48 h and 72 h in aged rats were (6.5±0.7) mm Hg and (8.9±1.0)mm Hg respectively, it was higher than that in non-aged rats 〔 (6.4±1.1) mm Hg and (4.2±1.2)mm Hg respectively 〕and in the control rats 〔(4.4±1.3)mm Hg〕,all P<0.05. However the value of HR, +dp/dtmax, -dp/dtmax were decreased significantly(P<0.05) in three experimentrl groups as well as in contol group. Conclusions The changes of hemodynamics and blood gas in the aged rats lasted longer than that in the adult rats. It might be related with the molecular network which the aged rats is different from that of the non-aged rats.
