颅内动脉瘤显微手术夹闭与介入治疗的对比研究 被引量:2

摘要 目的 对比研究颅内动脉瘤两种手术方法直接手术夹闭与介入治疗的优缺点。方法  2 16例颅内动脉瘤行手术治疗 ,其中 171例行了显微手术动脉瘤夹闭术 ,4 5例行了介入GDC栓塞术 ;对比研究两种手术方法的治疗效果、住院天数、费用等。结果 手术夹闭与介入治疗在治愈率、好转率及死亡率方面无显著差异 (χ2 =0 0 7,0 11,0 97;P均 >0 0 5 ) ,住院天数介入治疗明显少于手术治疗 (t=14 5 7,P <0 0 5 ) ,介入治疗费用明显高于手术治疗 ,两者差异显著 (t=2 6 1 81,P <0 0 1)。结论 显微手术夹闭与介入治疗均是颅内动脉瘤的有效治疗方法 ,两种手术治疗效果相近 。 Objective To study the cure result of endovascular ocluding with microsurgical operating to intracranial aneurysms.Method 216 cases of intracranial aneurysms were cured,171 cases of them were cured by microsurgical operating and the others were cure by endovascular occlusion with GDC.To analyze the cure result of the two methods about success rate,mortality,the days of being in hospital and the expense.Result The success rates,better rates and mortalities of this two methods are not obviously difference(χ 2=0.07,0.11,0.97;P>0.05).The days of being in hospital is more by microsurgical operating than by endovascular occluding,the difference being significant(t=14.51,P<0.05),but the expense was obviously lower by microsurgical operating than endovascular occluding(t=261.81,P<0.01).Conclusion Microsurgical operating and endovascular occluding are very effective methods to intracranial aneurysms,the success rates are similar.The days of being in hospital is less but the expense is much more by endovascular occluding in contrast with microsurgical operating.
