摘要
目的 :开发一套易于临床应用的计算机辅助正颌外科治疗设计及模拟预测评估系统 (CAPSPSOS)。方法 :利用扫描仪将患者X线头影测量片和侧面彩色照片输入计算机 ,根据正颌外科不同术式、不同部位的软硬组织移动比率关系 ,以MicrosoftVisualC ++6 0语言编译建立计算机辅助正颌外科治疗设计及模拟预测系统。对3 0例牙颌面畸形患者 (其中男性 11例 ,女性 19例 ,年龄在 18~ 3 0岁之间 )进行头影测量分析诊断 ,并模拟进行正颌手术术式和骨切开部位的确定 ,骨段移动的方向、距离以及需要切除骨量的预测 ,术后咬合关系的预测 ,术后颜面软组织侧貌的预测。将 3 0例患者的 14项测量参数的术后实际结果和术前预测结果测量参数进行配对t检验 ,分析比较结果。结果 :本系统对于 9项角度变化的预测均是准确的 ;对于 5项线距变化的预测有 4项是准确的。结论 :CAPSPSOS不仅可以模拟各种正颌外科手术 ,而且能够提供预测的患者术后侧面彩色照片 ,快速、形象、直观、准确 ,操作简单 ,具有临床使用价值。
Objective To develop and evaluate a Computer-assisted Planning and Simulation and Prediction System for Orthognathic Surgery (CAPSPSOS). Methods This software is programmed with Microsoft Visual C ++ 6.0. Thirty cases (11 of males and 19 of females,age 18 to 30 years old) with dentofacial deformities were included in this study. After pre-surgical cephalograms and color lateral photos of these patients were input to the computer and digitized, cephalometric analysis and simulative orthognathic surgery and predicted color lateral photos are processed by CAPAPSOS. Predicted pre-surgical parameters are compared with actual post-surgical parameters. Results Predicted and actual soft tissue profiles in this study are similar in all areas except for B’ point. Conclusion CAPSPSOS is of help for clinician in dentofacial deformity diagnosis and treatment planning. Predicted color lateral photos processed by CAPSPSOS are accurate and lifelike. The procedure is time-saving and easy to use.
