摘要 目的 了解不同胎龄期正常胎儿肺静脉血流频谱的特点。方法 分别在妊娠 2 0~ 2 4周、30~ 34周 ,应用胎儿超声心动图观察 10 0例正常胎儿心脏形态结构 ,系统探测了肺静脉血流 ,测量S波 (收缩期峰值速度 )、D波 (舒张期峰值速度 )、A谷 (心房收缩期峰值速度 ) ,并计算S/D比值。部分胎儿出生后 1周内随访 ,获得了相应肺静脉血流参数。结果 肺静脉S波、D波速度随着胎龄增加而增加 ,S/D比值始终大于 1。多数胎儿A波在多普勒波形上表现为前向较深的切迹 (A谷 ) ,有 2例胎儿肺静脉血流频谱未见A波显示。 4 8例胎儿出生后复查 ,肺静脉血流S波、D波速度值均明显高于胎儿期。 Objective To study pulmonary venous flow velocity waveform in normal fetus. Methods 100 pregnancies were prospectively studied by using fetal echocardiography for two times (fetus gestational age from 20 weeks to 24 weeks and from 30 to 34 weeks). Cardiac anatomic structure was visualized together with the presence of pulmonary venous flow wave patters. Peak systolic, peak diastolic, presystolic velocities were obtained and peak systolic/peak diastolic ratios were calculated. Some cases were followed up after birth by using echocardiography.Results Fetal pulmonary venous presystolic velocities of 2 cases were absent. Peak systolic and peak diastolic velocities were increased over the gestational age. Peak systolic, peak diastolic velocities were increased dramatically during neonatal period.Conclusion Pulmonary venous flow velocity waveform in normal fetus was different from that of adult.
