摘要
目的 观察丙泊酚靶控输注下对全身和脑代谢的影响。方法 实验犬 12只 ,基础麻醉后靶控输注丙泊酚 ,选取不同时间点测量全身二氧化碳产量 (VCO2 )、氧耗 ( VO2 )、全身代谢率(EE)、动脉和脑回流静脉血气、血糖、乳酸 ,计算动 静脉氧含量差 (Da jvO2 )、乳酸含量差 (Da jvL)、脑氧摄取率 (CEO2 )、脑糖摄取率 (Erglu) ,分析全身及脑代谢。 结果 靶控丙泊酚下全身及脑代谢均下降 ,且随丙泊酚血浓度升高 ,代谢进一步降低 ,并没有发现代谢失衡现象 ,而全身代谢与脑代谢呈一致性下降。结论 靶控输注丙泊酚能同时降低全身及脑代谢 ,无氧供需失衡发生 。
Objective To investigate effect of target-controlled infusion (TCI) of propofol on the systemic and cerebral metabolism. Methods A computer controlled infusion of propofol was performed with different concentrations in 12 dogs. Output of CO 2(VCO 2),consumption of O 2(O 2)and systemic metabolic rate (EE) were measured in different time points. Blood gas,blood sugar and lactic acid were analyzed. Arteriojugular venous oxygen content difference (Da-jvO 2)and lactic concentration difference (Da-jvL),cerebral oxygen and blood sugar extraction ratio (CEO 2,Erglu)were calculated for analysing the systemic and cerebral metabolic changes.Results The systemic and cerebral metabolism were descended during TCI,with higher concentration of propofol,they were descended more,and no metabolism unbalanced.Which systemic metabolism was down coincided with brain’s.Conclusion TCI of propofol may decrease the systemic and cerebral metabolism in a dose dependent manner,but balance of oxygen delivery and consumption is kept normal.
