摘要
本文报告手术治疗破入脑室的高血压性脑出血50例,占高血压性脑出血的41.7％。手术治疗结果优于保守治疗,其主要优点为:①手术清除脑实质及部分脑室内积血,解除脑受压;②术中放置脑室外引流管,有利于脑室内积血的清除,可解除脑脊液的循环障碍。早期手术可降低病死率。对手术指征、手术方法及影响预后的因素进行了探讨。
Fifty cases of hypertensive intracerebral hemorrhage with ventricular extension were. reported. These lesions accounted for 41.7% of all hypertensive intracerebral hemorrhage. The results of surgical treatment were better than that of conservative treatment. The main advantages of surgical treatment were as follows: (1) evacuation of intracerebral hematomas and partial hematonas in the ventricles for decompression of the cranial cavity; (2) continous ventricular irrigation and drainage were helpful to evacuate hematomas in the ventricles and to relieve obstruction of CSF pathways. With earlier operation, the lower fatality might be obtained. The indications of surgical treatment, operative procedures as well as influence of prognostic factors were discussed.
