原发性中枢神经系统血管炎的影像诊断(附7例报告并文献复习)

Radiological diagnosis of primary angitis of the central nervous system (report of 7 cases and review of literature)

目的 探讨原发性中枢神经系统血管炎的影像学诊断。方法 对 7例临床或病理证实的中枢神经系统血管炎的MRI、CT表现进行回顾性分析。结果 6例位于幕上 ,以额叶多见 ,其中 1例为双侧多发 ,幕下 1例位于小脑半球。影像学上 ,多数病灶边界模糊 ,累及灰质和白质 ,以白质为主 ,可有水肿 ,有不同程度斑片样或脑回样强化 ,部分类似肿瘤。结论 原发性中枢神经系统血管炎的影像学表现多种多样 。

Objective To investigate imaging features of primary angitis of the central nervous system (PACNS). Methods Seven cases with PACNS proved pathologically or clinically were included, and MRI and CT features of them were analyzed retrospectively. Results Supratentorial lesions were found in 6 cases, most lesions were in frontal lobe, and multiple lesions were found in 1 case, while cerebellar lesion was found in 1 case. On images, blurry margin was found in most lesions. The lesions involved both gray and white matter, but mostly in white matter. Edema was seen around some lesions. Patchy enhancement was found in most lesions. Some lesions mimicked tumor. Conclusion The imaging finding of PACNS is variable; CT and MRI play an important role in the diagnosis and follow-up of patients with PACNS.
