慢性心力衰竭患者血清瘦素水平

Serum leptin concentrations in patients with chronic congestive heart failure

目的 :分析慢性心力衰竭 (CHF)患者血清瘦素浓度变化。方法 :采用放射免疫平衡法检测 4 6例心功能NYHAⅠ级 :及 39例CHF患者 (即心功能NYHAⅡ～Ⅳ级患者 ,心功能Ⅱ级 14例 ,Ⅲ级 2 1例 ,Ⅳ级 4例 )血清瘦素浓度。结果 :血清瘦素浓度分别为 :心功能Ⅰ级组 14 .6 74± 6 .911μg/l;心功能Ⅱ级组 11.4 92± 5 .6 4 9μg/l;心功能Ⅲ级组 7.76 3± 3.32 1μg/l;心功能Ⅳ级组 6 .10 0± 2 .6 5 7μg/l;CHF组 9.0 18± 4 .5 19μg/l。与心功能Ⅰ级组比较 ,心功能Ⅲ级 ,Ⅳ级及CHF组血清瘦素浓度明显降低 ,具有统计学显著意义 (t=2 .185～ 2 .786 ,P <0 .0 5 ) ;与心功能Ⅱ级比较 ,心功能Ⅲ级及心功能Ⅳ级组血清瘦素浓度亦明显降低 ,亦具有统计学显著差异 (t=2 .6 0 5～ 2 .174 ,P <0 .0 5 ) ;各组中血清瘦素浓度降低与血清总胆固醇、甘油三酪、体重指数、左心室射血分数降低间的相关性呈显著统计学差异 (r =0 .5 6 4～ 0 .999,P <0 .0 5 )。结论

Objective:To explore the changes of serum leptin in chronic congestive heart failure(CHF).Methods:The serum levels of leptin in 46 patients with NYHA cardiac function class Ⅰ and in 39 patients with CHF(14 in class Ⅱ,21 in class Ⅲ and 4 in class Ⅳ)were assessed by radioimmunoassay.Results:The serum contents of leptin were 11.674±6.911μg/l in NYHA class Ⅰ group,11.492±5.649 μg/l in classⅡ group,7.763±3.321 μg/l in class Ⅲ group,6.100± 2.657 μg/l in class Ⅳ group and 9.018±4.519 μg/l in CHF group.The serum levels of leptin were significantly lower in class Ⅲ group,class Ⅳ group and CHF group,compared with class Ⅰ group,respectively( t =2.185～2.786, P <0.05).Compared with class Ⅱ group,the levels of leptin were also significantly lower in class Ⅲ group and class Ⅳ group,respectively( t =2.605～2.174, P <0.05).The decrease of serum leptin level was significantly correlated with the decreased serum total cholesterol level,triglyceride,body mass index and left ventricular ejection fraction in all groups,respectively( r =0.564～0.999, P <0.05).Conclusion:The role of the decreased serum leptin in CHF remains to be studied.
