幽门扩约肌切开成形在近端胃切除术中的应用体会

The practical experience of pyloric sphincter incision and figuration in the uppergastrectomy

目的:探讨幽门括约肌切开成形对于行近端胃切除术病人的应用价值。方法:回顾1998～2002年行近端胃切除术44例,比较幽门括约肌切开成形与未切开成形病人术后重度胃排空功能障碍的发生率以及术后消化不良症状随访情况。结果:本组21例无1例发生重度胃排空功能障碍、吻合口漏、腹腔感染并发症。对照组23例发生吻合口漏1例,重度胃排空功能障碍3例,术后消化不良症状随访本组病人较对照组病人饭后腹胀感觉轻,持续时间短。结论:幽门括约肌切开成形对于行近端胃切除的病人有应用价值。

Objective To study the practical erperience of pyloric sphincter incision and figuration for the patients who undertook the uppergastrectomy. Methods There were 44 patients received uppergastrectomy from 1998 to 2002,we compared the rate of severe gastricism and gastric empty dysfunction without pyloric incision with the patients who received uppergastrectomy plus pyloric sphincter incision and figuration in the up-gastrectomy Result There was no complication such as gastricism,leakage of .anastomosis and so on happened in all 21 patients ,one of the patients had leakage of anastomosis and three had severe gastricism in contrasting group the rate is 13%. Conclusion Uppergastrectomy plus pyloric sphincter incision and figuration is significantly helpfu to patients in decreasing the rate of gastricism and leakage of anastomosis after operation.
