细菌性阴道病实验室诊断方法评价

Evaluation of the experimental diagnostic methods for bacterial vaginosis

目的对几种常见的BV诊断试剂盒进行对比实验,选择一种能准确、有效诊断BV的最适试剂盒。方法随机选取我院妇科门诊患者80例,每位患者均一次性留取4份阴道分泌物标本,其中3份分别采用美国产BVBlue快速诊断试剂、国产BV试剂和胺实验试剂进行检测,另一份革兰染色,由经验丰富的技术人员依据Nugent评分标准评分,并结合临床症状对BV确诊。然后对三种试剂盒的检测结果与确诊结果之间的符合率进行比较。结果80例患者,其中40例被确诊为BV。美国产BVBlue快速诊断试剂、国产BV试剂、胺实验试剂与确诊结果的符合率分别为87.5%、65%、70%,美国产BVBlue快速诊断试剂符合率明显高于其他两种(P<0.05);此外,国产BV试剂和胺实验试剂假阳性率分别为25%、30%,明显高于美国产BVBlue快速诊断试剂(0)。结论美国产BVBlue诊断试剂快速、简便,假阳性率低,特异性、敏感性均符合临床要求,值得推广。

Objective To select a suitable reagent kit for the diagnosis of bacterial vaginosis(BV). Methods Eighty gynecologic outpatients were random selected. BV was determined respectively at the same time by 3 reagent kits, including BV Blue (made in America), BV reagent kit (made in China) and amine test reagent (made in China). Confirmed diagnosis of BV was according to Nugent standard and clinical symptoms. The corresponding rates of results were contrasted between 3 kits and confirmed diagnosis. Results In fourty among 80 BV patients, the results of BV Blue, BV reagent kit and amine test reagent were showed 87.5%, 65%, 70% in confirmed diagnosis respectively. The agreeing ratio of BV Blue was significantly higher than those of other two kits (P<0.005). In addition, the false positive rates of BV reagent kit and amine test reagent were 25%, 30% respectively, and showed significantly higher than that of BV Blue. Conclusions BV Blue is convenient, fast and have a lower false positive rate. Its particularity and sensitivity are in line with clinical request, so BV Blue is deserved to spread.
