多层螺旋CT测量肝体积的研究及在肝移植中的初步应用 被引量:4

摘要 目的研究多层螺旋CT(MSCT)测量肝体积的准确性及对肝移植的应用价值。方法(1)实验研究:MSCT扫描10只猪肝,层厚6.5mm,螺距1.25,重建间距3.2、6.5mm,半自动法重建肝脏三维模型,计算体积。(2)临床研究:对22例肝移植患者的肝脏术前行MSCT检查,重建间距6.5mm,余条件、测量方法同前。结果(1)实验研究:重建间距3.2、6.5mm测量肝体积的准确性分别为*(>1<L$M:NO、P(>1DL:19-Q,与实际肝体积有显著相关性(均RS:1(((,!I:1:$)。P&N临床研究:术前!"测量肝体积准确性为P((1DL(19NQ,与实际肝体积有显著相关性("S:1(>:,!I:1:$)。两名观察者测量结果间也有显著相关性("S:1(($,!I:1:$)。结论+,!"测量肝体积有较高的准确性和可重复性,是肝移植术前检查的重要组成部分。 Objective To study the accuracy of liver volume measurement u sing multiple-slice spiral CT(MSCT)and its value in liver transplantation. Metho ds In animal study, 10 pig livers were scanned by MSCT with collimation of 6.5m m, two reconstruction interval of 3.2mm,6.5mm, and pitch of 1.25. By semi-automa tic method, a 3D model was reconstructed to measure liver volume(CTLV). In clini cal study, 22 patients received MSCT scan with reconstruction interval of 6.5mm. Results In animal study the accuracy rate of CTLVs with interval of 3.2mm and 6.5mm were (97.5±1.0)% and (97.4±0.8)% respectively, which had a significant c orrelation with the actual liver volume(r=0.999,P<0.01). in clinical study the accuracy rate of CTLVs were (99.4±9.8)%. CTLVs measured by two observers had go od correlations with actual liver volumes (r=0.970 and 0.991, P<0.01 for both). Conclusion MSCT can assess liver volume correctly, and may be used as an essent ial step for preoperative evaluations of liver transplantation.
