大疱性系统性红斑狼疮1例

Systemic Lupus Erythematosus With the Manifestations of Bullous: A Case Report

分析报告 1例大疱性系统性红斑狼疮。患者以全身皮肤粘膜起红斑、水疱为表现起病 ,伴有口腔溃疡、双膝关节疼痛 ,尿蛋白 3 +,ANA阳性 1∶1 60颗粒型 ,ds DNA抗体阳性 ,ENA阳性、SS A阳性。取躯干部皮损行组织病理活检 ,HE染色示 :表皮下水疱 ,真皮浅层水肿 ,真皮乳头见中性粒细胞聚集 ,免疫病理示基底膜带可见IgG呈颗粒状沉积 ,符合大疱性系统性红斑狼疮病理改变。

Objective:We report a case of systemic lupus erythematosus with erythema, chronic vesicobullous lesions, oral ulcer and both knee joint pain. ANA(antinuclear antibody) and ds-DNA is presented in a high titer in the patient, and both ENA and SS-A were positive. Biopsy specimen revealed subepidermal blister. Indirect immunofluorescence was adopted for IgG test was showed the positive reaction. The clinical manisfestations and laboratory findings were the diagnosis of systemic lupus erythematosus.[
