局限期小细胞肺癌非手术综合治疗(附60例疗效与生存关系分析)

Nonsurgery Treatment of Limitied Small Cell Lung Carcinomas

治疗局限期小细胞肺癌60例,联合化疗加胸部^(60)Co 照射和随机分预防性脑照射及非脑照射。CR49例,PR4例,NR7例,总有效率88%。生存范围7～58个月,中位数18个月,2年生存者21例(35%),3年无瘤生存7例(11%)。生存时间与 KPS 计分、组织学亚型及治疗前 CEA 值和化疗周期次数相关,但与是否预防性脑照射无关。

Sixty patients with limited small cell lung cancer were treated by combined chemotherapy and ra- diotherapy.CR cases were divided at random into PCI group and non-PCI group.Combined chemother- apy regimen consisted of VP-16.Cisplatin.Adriamycine anti Endoxan.The therapeutic results were CR 49,PR 4,and NR 7,the overall response rate (CR+PR) was 88%.Survival range was 7～58 months and the median svrvival 18 months.21 patients survived more than 2years(35%),7 cases were free of disease and alive for more than 3years (11%).The surivival time was related to the KPS value.His- tologic subtyping,the value of pretreatment CEA and the cycles of chemothrapy,PCI didn't affect the surivival time of the patients.Severe myelosuppresion was observed during chemotherapy.
