膝骨性关节炎的关节镜治疗

Arthroscopic treatment of osteoarthristis of knee

目的 :探索临床分型指导下关节镜清理术治疗膝骨性关节炎的临床意义及效果。方法 :术前根据患者临床情况 ,将膝骨性关节炎分为滑膜炎型、髌股关节型、交锁弹响型、疼痛型和混合型。采用关节镜微创技术 ,进行针对性、局限化的手术治疗。结果 :41例获得随访 ,时间4～27月。参照刘玉杰等的疗效评价方法 ,优22例 ,良13例 ,可4例 ,差2例。优良率达85 4%。结论 :临床分型指导下关节镜术治疗膝骨性关节炎能够明确术中的治疗重点。可以有针对性的进行镜下处理 ,避免因过多的操作而加重对关节的干扰 ,影响疗效。

Objective:To investigate the clinical significance and effects of arthroscopic treatment of osteoarthristis of knee under guiding of clinical typing.Methods:The osteoarthristis of knee was divided into synovitis,patella-femoral joint,locking-clinicking,pain and mixture type according to clinical findings before operation.With arthroscopic micro-traumotic technique,the operations of pertinency and localization were performed.Results:41 cases were followed up for 4～27 months.The curative effects were evaluated by Liu Yujie's method.Of 41 patients with osteoarthristis of knee,22 were excellent,13 good,4 fair and 2 poor.The good rate was 85.4%.Conclusion:Arthroscopic treatment of osteoarthristis of knee under guiding of clinical typing plays an important role in finging the operative focal point and pertinency operation,decreasing the disturbance of knee joint and improving the curative effect.
