MR定位:伽玛刀治疗颅内动静脉畸形的评价

摘要 73例颅内动静脉畸形(AVM),MR定位、伽玛刀治疗和术后随访。全部定位图像病灶边缘清晰,距离检测精确度100%。其中11例(15%)头架固定点局部颅皮和颅骨信号消失;5例(7%)局部脑组织边缘出现浅弧样高或低信号影;余57例(78%)无伪影。按Spetzler-Martin法分类。术后6~23个月的过程中,AVM病巢闭塞率为:Ⅱ度83%,Ⅲ度54%,Ⅳ度35%,Ⅴ度25%,Ⅵ度50%。术后病灶周围水肿32例,患者无死亡。认为,MR定位是伽玛刀治疗颅内AVM安全、有效的方法。 MR was used in pre - operation diagnosis, stereotactic localization of gamma knife and follow -up after operation in 73 patients. It was very clear in margin of all localization image. The test of distance precision was 100%. The MR signal of local skull skin and bone was absent in the fixed point of head frame in 11(15%) cases. Except for this, there were shallow curve high or low signal in cerebral border in 5(7%) cases. The other 57(78%) patients had no any artifacts. Based on the Spctzler -Martin grade of AVM, the rate of AVM niduses obliterations were 83% in II ,54% in 111,35% in IV , 25%in V , 50% in VI during 6 to 23 monnths after operation. There were edema around the nidus in 32 cases. No death occured. We suggested that MR localization was safe, effective and reliable method in intracranial AVM treated with gamma knife.
