螺旋CT三期增强扫描在肝脓肿诊断中的价值

Triple-phase contrast enhanced CT:value in the diagnosis of hepatic abscess

目的 :评价螺旋CT三期增强扫描对肝脓肿的诊断价值。方法 :回顾性分析 2 8例经临床或病理证实的肝脓肿的螺旋CT平扫及三期增强扫描表现。三期延迟时间分别为 2 5s ,60s和 1 2 0s。结果 :肝脓肿增强扫描的典型CT表现为 :①动脉期灶周楔形或片状一过性强化 ;②门脉期显示环靶征 ,簇状征 ;③延迟期脓肿壁及水肿均为等密度 ,仅坏死区无强化呈低密度。结论 :螺旋CT三期增强扫描诊断肝脓肿具有特征性 ,是必不可少的检查方法。

Objective:To study spiral CT findings of hepatic abscess,and evaluate triple-phase enhanced spiral CT scan in the diagnosis of hepatic abscess.Method:Spiral CT findings of the plain scanning and triple-phase enhance scanning of the hepatic abscess in 28 cases were retrospectively analyzed.All of which were diagnosed clinically or pathologically.The delay times of the three phase were 25s,60s and 120s respectively.Result:The typical features of the hepatic abscess in contrast enhancement examination were as following:①Periabscess wedge-shaped or patchy enhanced area in arterial phase;②Double-target sign and cluster sign were shown in portal phase;③In the delayed phase,the wall and edematous zone of the abscess were all isodense,only the necrotic areas were not enhanced and was hypodense.Conclusion:Triple-phase contrast enhancing SCT scanning was specific in diagnosing hepatic abscess and was an indispensable examination method.
