肾细胞癌中p53基因表达与预后关系的研究

Studies on the relation of the prognosis and the p53 gene expression in carcinomas of kidney

目的 研究肾细胞癌中p53基因表达与预后的关系。方法 采用S-P免疫组化法对76例肾细胞癌根治术蜡块标本进行标记,观察p53基因表达情况。结果 p53基因阳性表达者28例,阳性率为36.8％(28/76)。其中透明细胞癌p53基因阳性表达率为38％(19/50):颗粒细胞癌p53基因阳性表达率为35.7％(5/14);混合细胞癌p53基因阳性表达率为33.3％(4/12)(P>0.05)。临床分期Ⅰ期p53基因阳性表达率为30.8％(8/2);Ⅱ期p53基因阳性表达率42.8％(12/28);Ⅲ期p53基因阳性表达率为41％(5,12);Ⅳ期p63基因阳性表达率为30％(3/10)(P>0.05)。p53基因阳性表达者5年生存率为31.8％(7/22);p53基因阴性表达者5年生存率为75％(21/28)(P<0.05)。结论 p53基因表达变化对临床判断肾细胞癌的预后有一定的参考价值。

Objective To study the relation of the prognosis and the p53 gene expression in carcinomas of kidney. Methods The p53 gene expression in carcinomas of kidney of 76 cases were observed by S - P immune-histochemistry methods. Results The positive rate of the p53 gene exprssion in 76 cases of carcinomas of kidney was 36.8 % (28/76) . The positive rate of the p53 gene expression in carcinemas of lucency cell, granule cell and both the lucency and granule cell, were individually 38% (19/50), 35.7% (5/14) and 33.3% (4/ 12). The positive rate of the p53 gene expression in carcinomas of clinic phases of Ⅰ,Ⅱ ,Ⅲ and Ⅳ, were individually 30.8%(8/26) , 42.8%(l2/28), 41%(5/12) and 30%(3/10)(P>0.05). The rate of 5 - year -survival with positive expression of the p53 gene (31.8% ) is lower than that with negative expression of the p53 gene (75% )( P < 0.05) . Conclusion The changes of expression of the p5; gene play important roles on the diagnosis of the prognosis of the patients with carcinomas of kidney.
