108例慢性乙型肝炎患者外周血单个核细胞IFN-γ、IL-4 mRNA表达与中医证型的关系

Relation between mRAN Expression of IFN-γ and IL-4 of Monocyte in the Peripheral Blood of 108 Patients with Chronic Hepatitis B and the Syndrome Types of TCM

目的 :探讨慢性乙型肝炎中医临床分型与细胞因子mRNA表达之间的关系。方法 :对 10 8例不同中医证型的慢性乙型肝炎患者外周血单个核细胞中IFN γ、IL 4mRNA的表达进行了检测。结果 :慢性乙型肝炎 5证型患者按湿热中阻→肝郁脾虚→瘀血阻络→肝肾阴虚→脾肾阳虚的顺序 ,IFN γ水平逐渐递减 ,而IL 4水平则逐渐升高 ;细胞免疫应答减弱 ,病毒不能完全清除 ,病情迁延不愈 ,病性由实致虚 ,病势由浅入深。结论

Objective:To evaluate the relation between the clinical syndrome types of TCM of chronic hepatitis B and mRNA expression of cytokines.Methods:The mRNA expression of IFN-γ and IL-4 in the monocytes of peripheral blood of 108 patients with chronic hepatitis B of different syndrome types of TCM was determinated. Results:It showed that the degree of IFN-γ of these five syndrome types of patients with chronic hepatitis decreased gradually in the order of damp-heat blockage, liver stagnation and spleen deficiency, collateral obstruction by blood stasis,Yin deficiency of liver and kidney, and Yang deficiency of liver and kidney, while the degree of IL-4 raised gradually in the same order. The response of cell immunity declined, and the virus could not be clear away completely. The disease-process lasted for a long time, and the disease property changed from excess to deficiency, the disease trend changed from superficial to deep.Conclusions:The mRNA expression of cytokines provides the evidence for the objectivity of TCM chronic hepatitis B.
