黄色肉芽肿性胆囊炎56例临床病理研究

Clinicopathological Study of 56 Cases of Xanthogranulmatous Cholecystitis

目的 探讨黄色肉芽肿性胆囊炎 (XGC)的临床病理特点。方法 对 5 6例经病理诊断证实的XGC进行常规HE切片观察 ,3例做免疫组化染色。结果 5 6例XGC的临床表现与普通型胆囊炎相似 ,缺乏特异性。病理眼观见胆囊壁黄色结节或肿块 ,显微镜下显示在急慢性炎症背景的基础上 ,胆囊壁形成肉芽肿性病变 ,以泡沫状组织细胞聚集为主。结论 XGC是一种特殊的肉芽肿性炎症 ,发病率居胆囊疾病的第二位。某些囊壁结节或包块与周围组织和器官粘连并形成内瘘。其正确诊断有赖于病理学检查。

Objective To investigate the clinical and pathological characteristics of Xanthogranulmatous cholecystitis(XGC).Methods Pathological HE sections were reviewed in 56 cases of XGC and inmmunohistochemical stain was used for 3 cases.Results 56 cases of XGC were short of peculiar clinical manifestation and similar to common cholestitis.Pathological observation with the naked showed that there were yellow tubercles or tumors on the cholecystitis.Microscopical examination showed that foam cell granuloma on the background of the severe acute and chronic inflammation was the characteristic pattern.Conclusions XGC is a special granulomatous inflammation;attack rate of XGC was the second in cholecystic disease.Some tubercles or tumors were subject to adhere to the surrounding tissue or organs and to form an internal fistula.Right diagnosis of XGC relies on pathologic test.A differential diagnosis from poorly differentiated gallbladder carcinoma should be made to some XGC cases.
