新生儿血清甲状腺激素的动态观察

DYNAMIC OBSERVATION OF THE LEVELS OF THYROID HORMONE IN THE SERUM OF NEWBORNS

目的 观察新生儿脐血及出生后 5d内血清甲状腺激素的变化 ,探讨其生理意义。方法 应用放射免疫分析和免疫放射分析法对 4 0例新生儿脐血及出生后第 2 4、4 8、72、96、12 0h的血清T3 、T4、rT3 和TSH进行了检测。结果 与健康成人相比 ,新生儿脐血呈低T3 、高rT3 和高TSH血症。一旦分娩 ,血清TSH值急剧上升 ,继而T3 、T4值也产生波动。生后第 5天血清TSH、T3 已达健康儿水平 ,T4也明显下降。血清rT3 呈持续高值。结论 了解早期新生儿血清甲状腺激素变化特征 ,对诊断新生儿先天性甲状腺功能减退和判断各种病理状态下甲状腺功能有重要意义。

Objective To observe the changes in the levels of thyroid hormone in the serum of the cord blood of newborns in the first five days after delivery.Methods The levels of T 3,T 4,rT 3 and TSH in the serum of newborns in the first five days after delivery were detected through radioimmunoassay and immunoradiometric assay.Results The levels of rT 3 and TSH were found higher,but the level of T 3 was found lower than those in healthy adults.The level of TSH went up greatly and the levels of T 3 and T 4 also changed a lot.The levels of TSH and T 3 returned to normal levels and the level of T 4 decreased as well,while the level of rT 3 remained at high level.Conclusion The detection of the levels of thyroid hormone can help to diagnose congenital hypothyroidism and to effectively evaluate the functions of thyroid hormone under the pathological conditions.
