经纤支镜微波治疗晚期中央型肺癌的临床价值 被引量:4

摘要 目的 :探讨纤维支气管镜下微波组织凝固法治疗晚期中央型肺癌的价值。方法 :8例肺癌患者按纤支镜介入治疗常规准备。插入纤支镜到达病变部位后 ,将微波同轴天线经纤支镜活检孔插入 ,尖端插入或紧贴治疗部位。调节微波功率为 4 5~ 6 0W ,持续时间为 8~ 15s/次。行多点烧灼。同时配合用鳄口钳清除焦化坏死或游离的组织。 1次 / 5~ 7d ,3~ 5次为一疗程。结果 :显效 :纤支镜下见瘤体缩小 6 0 %以上 6例 ;有效 :瘤体缩小 >30 %≤6 0 %者 2例 ,气逼等临床症状均有明显改善。结论 :经纤维支气管镜微波治疗晚期中央型肺癌为一种操作简单、安全。 Objective:To study the value of tissue coagulation by microwave via bronchofibroscope in the treatment of advanced central type pulmonary carcinoma.Methods:In 8 patients with lung cancer bronchofibroscopes were inserted to the locus of cancers.The microwave aerials with same axle were then inserted through bronchofibroscope's duct.The tip of microwave aerial was inserted or closed to the locus of cancers.The power was modulate from 45 W to 60 W,continued 8~15S every time.Many spots were burned.Necrotic or dissociative tissues were cleared by the clip.Every patient was treated a time every 5~7days.A course included 3~5 times.Results:Obvious effect: the size of tumor was shrunk by more than 60% in 6 cases; common effect: the size of tumor was shrunk by more than 30% but less than 60% in 2 cases.Breath difficult and other symptoms were alleviated obviously.Conclusion:Microwave tissue coagulation(MTC)through bronchofibroscope may be a transient way to treat advanced central type pulmonary carcinoma. It can be operated easily and safely, and has quick effect.
