经支气管针吸术在腔外型肺癌诊断中的价值(附56例分析)

The value of TBNA in the diagnosis of exophytic or submucosal and peribronchial lung cancer:Report of 56 cases

目的 探讨经支气管针吸术 (TBNA)在腔外型肺癌诊断中的价值。方法 回顾性分析了 5 6例腔外型肺癌患者应用纤支镜进行 TBNA、活检和刷检检查的情况 ,并进行对照研究。结果 5 6例腔外型肺癌患者中活检确诊 30例 ,刷检确诊 2 2例 ,TBNA确诊 4 4例 ,三者阳性率分别为 5 3.6 %、 39.3%和 78.6 % ,它们之间的差异有显著性 (P<0 .0 1) ,TBNA诊断率明显高于活检、刷检。结合纤支镜下征象进一步分析 ,TBNA对管壁外压性膨隆者较活检和刷检更有诊断优势。结论 TBNA有助于提高腔外型肺癌的诊断率 ,具有较高的临床应用价值。

Objective To evaluate the value of transbronchial needle aspiration (TBNA)in the diagnosis of lung cancer which presenting as either an exophytic mass lesion or submucosal and peribronchial lesion.Methods The results of TBNA,transbronchial biopsy,bronchial brushing in fifty-six lung cancer patients were analyzed retrospectively.Results In fifty-six lung cancer patients,TBNA showed a positive result in 78.6% of cases,in comparison to 53.6% for transbronchial biopsy,39.3% for bronchial washing(P<0.01).In the exophytic mass lesion,TBNA was preferable to increased the diagnostic yield.Conclusion TBNA is an effective bronchoscopic sampling technique,it can improve the diagnosis rate of lung cancer.
