黄色肉芽肿性胆囊炎10例临床病理分析

Xanthogranulomatous cholecytitis:A clinicopathological study of 10 cases

目的 探讨黄色肉芽肿性胆囊炎 (XCG)的临床病理特征。方法 回顾性分析我院 2 0 0 1～ 2 0 0 4年间经病理确诊的 10例黄色肉芽肿性胆囊炎。结果 XCG与普通胆囊炎临床表现相似 ,常反复发作。大体检查见胆囊壁常呈不同程度增厚 ,大多数囊壁内可见黄色或棕黄色结节 ,直径 0 .5～ 1.0 cm。显微镜观察见胆囊壁结构常被破坏 ,代之以由大量泡沫细胞、纤维母细胞及急慢性炎症细胞组成的肉芽肿 ,并可伴有多核巨细胞。结论 XCG在临床上与普通胆囊炎、胆囊癌难以鉴别 ,确诊有待病理。

Objective To explore the clinicopathological features of Xanthogranulomatous cholecytitis(XCG).Methods Retrospective analysis was made in 10 cases of XCG from 2001 to 2004 in this hospital.Results The clinic symptoms of the patients with XCG was similar to that of the ordinary chronic cholecystitis.Grossly,there were yellow or brown nodular in the wall of gallbladder which diameter between 0.5 to 1.0 cm.Microscopically,granulomas consist of foamy histocytes,fibroblasts,acute and chronic inflammatory cells were found in the gallbladders walls which were destroyed in varying degrees.Conclusions It may be difficult to distinguish XCG from ordinary chronic cholecystitis and carcinoma in the gallbladder in clinic.Pathological diagonosis is always needed in this disease.
