GGTⅡ特异膜免疫吸附检测法的建立

摘要 目的 使用特异性抗体和植物血凝素 (PHA)建立γ 谷氨酰转肽酶同功酶Ⅱ (γ glutamyltranspeptidaseisoenzymesⅡ ,GGTⅡ )特异膜免疫吸附检测法 ,探讨其对肝细胞癌 (HCC)的临床价值。方法 从HCC患者血清中提取、分离、纯化GGTⅡ ,经神经氨酸酶 (NMD)消化后 ,制备GGTⅡ特异性抗体 ,将纯化的特异性抗体 (anti GGTⅡ )、NMD和PHA吸附并固定到硝酸纤维素膜上 ,对HCC及其相关疾病患者血清中GGTⅡ进行定性检测。结果 本法检测HCC阳性率达 86 .6 % ,特异性为 95 .7% ;对直径小于5cm的HCC检测阳性率达 6 0 .6 % ,对AFP阴性或低浓度的HCC患者检测阳性率为 6 6 .2 %。结论 GGTⅡ特异膜免疫吸附检测法可作为AFP的补充用于HCC的辅助诊断和早期发现。有简便快速、微量灵敏和价格低廉的特点。 Objective To establish a new specific immuno-membrane adsorption method for the detection of the gamma-glutamyl transpeptidase isoenzymes Ⅱ(GGT Ⅱ)activity with anti-GGT Ⅱ antibody and phytohemagglutinin (PHA) and evaluate its diagnostic value for hepatocellular carcinoma(HCC).Methods GGT Ⅱ was separated and purified from serum of patients with HCC and rabbits were immunized with purified human GGT Ⅱ.The activity of GGT Ⅱ in serum of patients with HCC or control patients with other diseases was detected by nitrate cellulose membrane which had adsorbed purified anti-GGT Ⅱ antibody,neuramidinase (NMD) and PHA.Results The method had sensitivity of 86.55% and specificity of 95.66% for HCC.The positive rates for HCC (<5 cm) and HCC with AFP-negative or low AFP concentration were 60.61% and 66.15% respectively.Conclusion The established immunoadsorbent membrane assay is a rapid,simple,sensitive and specific method for the diagnosis of HCC and can be used as a supplementary marker for HCC.
