广西壮族自治区乙型肝炎疫苗纳入儿童计划免疫血清流行病学本底调查

A Baseline Survey on Serological Epidemiology for the Integration of Hepatitis B Vaccine into the Expended program on Immunization in Guangxi

为广西壮族自治区将乙型肝炎 (乙肝 )疫苗纳入儿童计划免疫的实施提供本底资料 ,采用多阶段整群系统随机抽样方法 ,于 2 0 0 3年调查了 0～ 3岁儿童 5 6 70人 ,儿童母亲 12 16人。结果显示 :0～ 3岁儿童乙肝疫苗接种率、全程接种率和首针及时接种率分别为 76 16 %、73 4 4 %、32 6 6 % ;乙肝病毒表面抗原 (HBsAg)与抗体 (抗 HBs)、乙肝病毒核心抗体 (抗 HBc)阳性率分别为 3 37%、4 0 18%、5 17%。与 1992年调查结果相比较 ,乙肝疫苗接种率提高了 77 6 1% ,HBsAg携带率由 9 6 8%降至 3 37% ,保护率为 6 5 19%。首针及时接种、全程接种乙肝疫苗儿童的HBsAg、抗 HBc阳性率非常显著地低于未接种的儿童 ,HBsAg阳性母亲儿童的HBsAg、抗 HBc阳性率非常显著地高于其他儿童。提高首针及时接种率、全程接种率 ,降低阳性母亲儿童的HBsAg、抗

To get baseline data for the implementation of the integration of hepatitis B vaccine(HepBV)into the Expended Program on Immunization,a multistage cluster-randomized sampling was designed to survey 5 670 children aged 0～3 years and 1 216 mothers in 2003 The results revealed that the HepBV coverage,full dose coverage and the first dose timely coverage among the children were 76 16%,73 44% and 32 66% respectively;the HBsAg positive rate,anti-HBsAg positive rate and anti-HBc positive rate were 3.37%,40.18% and 5.17% respectively. As compared with the survey in 1992,HepBV coverage increased 77 61% and the HBsAg carrier rate dropped from 9 86% to 3 37%, the protection rate was 65 19% The HBsAg positive rate and anti-HBc positive rate among the children who had received a timely birth dose and full dose vaccination were observed to be considerably lower than those among the unvaccinated children Among the children of the HBsAg positive the HBsAg positive rate and anti-HBc positive rate were significantly higher than those among other children The hepatitis B control should focus on improving the first dose timely coverage and full dose coverage and also on decreasing the HBsAg positive rate and anti-HBc positive rate among the children whose mothers are HBsAg positive
