摘要
目的研究正常成年男性血清PSA随年龄变化的关系及其意义。方法检测不同年龄段696例正常男性血清总PSA(TPSA)、游离PSA(FPSA)浓度,计算F/T比值;同时检测154例良性前列腺疾病患者和69例前列腺癌患者血清TPSA,FPSA的水平及F/T比值,并与正常男性比较。结果在正常成年男性中,年龄与血清中TPSA,FPSA显著相关(P<0.01),有随年龄而增加的趋势,F/T比值虽有一定的波动,但与年龄无相关性(P>0.05)。154例良性前列腺疾病患者TPSA,FPSA及F/T比值分别为(8.80±16.16)μg/L,(1.67±2.82)μg/L,(0.21±0.06);69例前列腺癌患者TRSA,FPSA分别为(136.09±198.02)μg/L,(13.74±24.11)ug/L,明显高于正常男性(P<0.01),F/T比值为(0.11±0.03),明显低于正常男性(P<0.01)。结论不同年龄段正常男性PSA应采用不同的正常参考范围,应用特定年龄范围结合检测TPSA、FPSA及F/T比值,可提高前列腺疾病的诊断特异性和敏感性,对前列腺癌的鉴别诊断有重要意义。
Objective: To study the correlation and clinical significance between prostate specific antigen and age in the healthy males.Method: In this study, three groups including 696 healthy males (19 to 88 years old), 154 patients with benign prostate disease, of which 69 patients with the prostate cancer were included. Total, free and the ratio free to total PSA were determined for each studied participant.Result: Total and free PSA correlated directly with patients' age( r = 0.446 , r = 0.426 respectively). In 154 patients with benign prostate disease, total, free PSA and the ratio were ( 8.80 ± 16.16 )μg/L,( 1.67 ± 2.82 )μg/L,( 0.21 ± 0.06 ),respectively;while in 69 patients with prostate cancer patients, total, free PSA and the ratio were ( 136.09 ± 198.02 )μg/L,( 13.74 ± 24.11 )μg/L, ( 0.11 ± 0.03 ) respectively,and the differences were obvious.Conclusion: The establishment of appropriate reference ranges for total, free PSA in healthy males with different age as well as the ratio free to total will increase the specificity and sensitivity in diagnosing the prostate disease,and has an important effect on the diagnosis of prostate cancer.
