门脉高压症预防性断流术17例观察

Review to the prophylactive pericardial devascularization for the treatment of 17 cases with portal hypertension

目的:探讨门静脉高压症预防性断流术的可行性及效果。方法:对17例门脉高压症、肝功能为childA、B级,胃镜提示曲张静脉的直径>3 mm均存在红色征的患者施行择期预防性断流术,并对术后疗效及上消化道出血的预防进行追踪观察。结果:17例病人术后均取得满意疗效,随访1-7年,均未发生上消化道出血,且肝功能均有明显改善。结论:门脉高压症预防性断流术比较安全,疗效好,避免了急症手术死亡率高,合并症多的缺点。

Objective : To review our experinces and evaluate the efficacy of prophylactive pericardial devascularization with splenectomy for the treatment of portal hypertension. Methods : 17 patients with portal hypertension were treated with this operation. The severity of the liver disease was graded according to the Child's criteria: class A (12 patients), class B (5 patients) .Diameter of esophagogastric varices > 3 mm with red color sign were observed by endoscope of all patients. Results : The follow-up time was 1 - 7 years. No recurrent bleeding was found in all patients. And the liver function were amelioration obviously. Conclusion : The prophylactive pericardial devascularization with splenectomy was safer and effective choice for the treatment of portal hypertension with serve esophagogastric varices, avoiding high mortality rate and Postoperative complication of emergency operation.
